Icons missing in jomsocial menu, module title style messed u

Please help, the icons on the jomsocial menu are missing, please see attached images.

Also, the title bar on the modules in jomsocial is not showing properly, please help, see images.

One IMPORTANT question; where can I find full instructions to upgrade, which files to upgrade, just the rest files for jomsocial theme or the whole template files?
