Problem with Single Contact

Hello, I am a beginner and have a problem. I want to create a single contact. It Should look like , on the guck page (template: theRealdesign) (photo 2), but it looks like this (photo 1). Why is that? Where can I make adjustments?

There's no special requirements
It's just a question of populating Contact Details parameters and enable the ones that you want to provide.

Here's a suggestion:
You can install a local server on your pc, like for example xammp and install the quickstart package that show exactly as our demo. Them you can make tests and take the best acquaintance of it.
