Register Now Contact Form

Hi -

I want folks who click the "Register Now" button to have to fill out a contact form (including "Name", "Email", and "College/University Affiliation" as fields) that automatically emails that info to me at [email protected]

I have no idea how to do this - any help would be greatly appreciated.
