Location of Contact Form

Hi, i want to know were is located the Contact Form that is in bottom position.

Can You please show me a screen of the form You have in mind ? I see no contact other than published with Custom Module HTML in this template.

Hi, i want to use that same form the template brings. I found the Custom HMTL but the content of that module brings only the name of the form. I want to know in which place of the administrator site is located that form to add more fields to that form. I attach you screen shot of the form i'm looking for.

This is a creativity template Anyway this form is a Custom HTML module, so form will not work since it's not attached to any extension, but You can replace it with any contact module available here: http://extensions.joomla.org/extensions ... tact-forms
