Offline page - Password/Username Dialogue Error Box

Hey

I've just installed this theme. Whilst I'm building the site I have set it to be in offline-mode, ie: show the offline page. I've very slightly altered the offline page with a new logo and text.

However, since I know people will try entering a user name and pass just for the hell of it. I'm concerned about the way the dialogue box (or the lack of a dialogue box): as you can see in the image:

I can manually fix this, though I would have thought it should not be behaving like this. Is it an error/corruption or an oversight by Gavick?

Also what's the best way to fix it and which CSS file should I edit/fix this in?

I've managed to fix this by replacing my version 3.6.0 to 3.6.2 however, the little "x" close button still does not function!
