terms of service checkbox on registraiton

Would like a terms of service checkbox when a user first signs up to the website. Is there any way to add this to the registration process?

I am getting this error when I click on TOS to see if the article is linked right:

: "Notice: Undefined variable: GK_TEMPLATE_MODULE_STYLES in C:\xampp\htdocs\xampp\uftest\templates\gk_startup\component.php on line 22"

Is it because I'm using xampp?

I've checked this in different template and I didn't seen Waring notice.
