Problem with contact form in Creativity theme

I'm trying to use this shortcode for the contact form within a text widget:

[contactform name="yes" email="yes" title="yes" captcha="yes" mailto="[email protected]"]

Problem #1 is that I am not receiving the email. Once the "Submit" button is clicked, the index page is reloaded. I am given no confirmation or notice the email was sent (or failed).

Problem #2 is that the captcha does not show up.

Captcha will work only if the reCAPTCHA keys are provided in the theme advanced settings.

only thing that works is if I replace with code from contact us page (generates from contact form button)--

[contact-form][contact-field label='Name' type='name' required='1'/][contact-field label='Email' type='email' required='1'/][contact-field label='Comment' type='textarea' required='1'/][/contact-form]

However; this re-positions the text to the left and email form to the right. Guess I'll have to just deal with this look?

Unfortunately it's not possible to change the layout using the shortcode - in this case you'll have to change css.
