How can apply in Music State template the Contact form that I can see here in http://demo.gavick.com/joomla25/musicstate/index.php/pages/contacts? Is there a plugin or module for this?

Hi,

You only need to make a contact category published, and a contact published in that category, then make a menu (contacts > a single contact) and you choose the contact you have created first.
