Remove image from contact form

hi,

please check my contact form below, and i am using quickstart package of instyle

http://www.samratbazaar.com.sg/index.php/contact-us

how to fill in something for the image. dont seem to find how to insert the image. optionally if i find this image slows the loading of the form, how to remove it

Just clear the selected image inc contact settings and it will simply disappear.
