Page titles

I'm confused about the double titles in Game. I'm using K2 mainly and When I link directly to an item the title of the item appears both in the "gkHeader" position and "header" positions.

In the demo site the gkheader only ever seems to show the category title and not the item title. How is this achieved?

I assume this is related to the menu setup but I can't seem to get it to work..
