GK-contact plug in:needed some change in form

Hi
1- I need to add more item such as telephone and company name in GK-contact form. How can i do that?
2- I tried to set 'on' CAPTCHA but it doesn't show in the form.
3- How can i have required option (with * sign that shows the field mus be filled) to avoid getting garbage messages?

I have one advice for you, use typical form creator (module or component) and don't waste your time with php customizations.
Gk c0ntact plugin is very basic solution for beginners not for advanced purposes.

if i use another one, will it be match with my template(John)? Have u any recommendation?

So I think you should use module.
Probably if extension was good designed you have two option use default style from template or use build-in theme.
There are many of them. In my projects I use commercials solutions, for example RSForms.
