How to do things like "Games" Menu Item and other thing

First Let me congratulate you for your excellent work.

I will like to know two things.

1. How to configurate this "Item Menu"

2. How to add the titles to the "contact form fields" like you have.

I will like to add a third question.

3. How did you configured the other "Menu items" without the Image show 4 header, for example for the link http://demo.gavick.com/joomla25/game/index.php/features there is just a background picture. NO the Image show.

I supposed that you have a template copy for the other menu items, but how to configured to not show the "Image show 4" and show the "background image"
