Header reservation form problem

I've followed documentation instructions in order to display reservation form in the header but the form is not being displayed.

The shortcode I am using is:

Do you have GK Hotel Reservation plugin activated? Could you please provide me with a URL to your website and back-end access via PM (click the “Private Message” text underneath my avatar) so that I may analyze it?

Your header slider page (first subpage of the frontpage) should contain custom field:
page_function with the value header-slider
(it's imported by default). Then the shortcode (and the slider) work properly. I've already added this custom field.
